Your First Visit

When you arrive, check in at the front desk. You will receive new-patient paperwork: a health questionnaire, a request for your insurance information, your contact details, and information about our office and how we handle your privacy. You can save time by downloading and completing the forms below before you come.

Your initial dental exam will take around one hour. We will ask about your oral hygiene routine and any health concerns and show you proper tooth care. A hygienist cleans your teeth, then Dr. Singh completes the exam and recommends treatment. If something needs fixing, many treatments can be done the same day.

Print and sign the forms that apply to you and bring them to your appointment. New patients need the New Patient Form. Existing patients updating their records only need the short Health History form. Everyone receives our HIPAA privacy acknowledgement.

Pain and Comfort

We work to keep discomfort to a minimum. Before any procedure, Dr. Singh talks with you about your pain tolerance and creates a pain management plan for you. If you have dental anxiety, say so. We take our time, explain each step, and go at your pace. Ask us about comfort options.

Dental Insurance

We accept and work well with most insurance plans. Bring your card or plan details to your first visit, and we will help verify your benefits and estimate your portion before treatment. If your insurance does not cover all of your treatment, we will help you find a payment solution. We are health care professionals first, and we want you to get the care you need.

Financial Policy and Financing

We serve families throughout the Glendale area and work with patients on payment, with or without insurance. We accept Visa, MasterCard, Discover, American Express, Apple Pay, cash and check. For larger treatment, CareCredit, Cherry and LendingClub offer monthly payment plans, and our front office can help you apply.

Appointment Policy

If your plans change, please give us at least 48 business hours' notice to reschedule or cancel so we can offer your time to another patient. A missed appointment carries a $75 fee to reschedule. New patients pay a $59 fee before scheduling; it is refunded or applied to treatment as described on our Specials page, and it is non-refundable if the appointment is missed.

   Download, print and sign the forms above, or arrive a few minutes early and complete them at the front desk.

   Exam, X-rays and cleaning

   Your first visit takes about an hour: digital X-rays, a cleaning with your hygienist, and a full exam with Dr. Singh.

   Your plan

   Dr. Singh explains what he found, your options and the costs. If treatment is needed, we can often start the same day.
